Wheelchair alert changed the method of mounting the castor assembly to the sideframe of the Action 2000 wheelchair.
The thread length on the castor mounting bolts was subsequently found to be insufficient to guarantee that the bolts could be correctly tightened and/or remain tight in use. This problem was corrected in August 2008.
Following reports of failure and subsequent falling of the user, Invacare has issued a Field Safety Notice and the Medicines and Healthcare products Regulatory Agency an alert.
It states that all Action 2000 wheelchairs with serial numbers from 7007533957 to 7008568013 inclusive must have replacement bolt kits, reference 1528509 from Invacare (free of charge) fitted in accordance with the instructions.
